Birth, Marriage, Divorce and Death Vitals Records

Certified copies of birth, marriage, divorce and death certificates can be obtained at the Mont Vernon Town Clerks Office in person or online.

New Hampshire vital records are considered to be private and access to them is restricted by statute to those individuals who have a "direct and tangible interest" in the record. This includes: the applicant; a member of their immediate family (by blood or marriage); their guardian or legal representative.

In order to obtain a copy, a request form must be filled out.

Anyone requesting a certified copy of a record must present positive identification including: a driver's license; passport or other picture identification; or, in the absence of acceptable picture identification shall complete this state form.

Submit your request form, payment and proof of identification in person or by mail to the Town Clerk's office.
