The following services and permits are available from the Police Department. Some may require a fee.

Request for Report

When people are involved in a traffic accident, victimized by thieves, or suffer property damage, very often they need to obtain a copy of the police report. The copy may be needed for insurance purposes, for legal purposes or litigation, or for proof that the incident was reported. You can fill in this Request for Report and either drop it off at the Police Department or mail it in. There is a $20.00 fee for all requested Police Report copies, except for attorney discovery requests. This fee is payable to the "Mont Vernon Police Department" and may be paid by Cash, Personal Check, Bank Check or Money Order.

House Check Request & Special House Check Request

Use this House Check Form to request that the police check your house periodically while you are away from your residence for an extended period of time. 

Use this Special House Check Form to request that the police check your house periodically while you are away from your residence AND you will be leaving your children at home. This is typically requested by parents to prevent parties and unwanted visitors while they are away. This also give the police permission to enter the residence as appropriate.

If you do deliver either form to the police department and no one is at the office, you can slide it under the office door.  You may also email the forms to us.

Good Day Mont Vernon Request

Our purpose in the “Good Day Mont Vernon” program is to provide persons who live alone a secure feeling in knowing that someone is in contact with them daily and will provide them with help when needed.

It is requested that you call the police department each morning between 8:00 a.m. and 10:00 a.m. to check in. If we do not hear from you by 10:00 a.m., we will call you to make sure everything is all right.

If we still cannot reach you, we will send a police officer to your home to check on you.

If you are interested in being a part of our “Good Day Mont Vernon” program, please print out and fill out the Good Day Mont Vernon Request Form and return it to the police department. It is very important that you provide us with all of the information requested so that we can contact the necessary persons in the event of an emergency.

Alarm Registration

We support residents of the community that choose to have alarms installed in their home or business. Please complete and return to the police department an alarm registration form so that we can contact you should we respond to your home/business for an activation.  This allows us to notify you of the outcome of our response and provides us a list of additional people that we can contact on your behalf in case of an emergency.

Fireworks Display Permit

The Town of Mont Vernon requires a written permit for persons wishing to display fireworks. A fireworks display permit needs to be completed and submitted to the Chief of Police for processing. The town ordinance and requirements for fireworks displays are attached to the display permit. If you have any questions, please don't hesitate to contact us.

Alzheimer's/Dementia Alert

This voluntary form is for families to fill out so police can obtain critical information that may assist with calls for service involving people with Alzheimer's/Dementia.

Persons with Development Disabilities Checklist for First Responders

This voluntary form is to assist first responders when responding to calls involving people with developmental disabilites. The information contained on the form will be provided to police, fire, and EMS. This is completely voluntary and confidential.

Hawkers and Peddler's Permit

Per Mont Vernon Town Ordinance labeled "Hawkers and Peddlers," anyone who wants to sell services or products door-to-door must obtain a permit from the Mont Vernon Police Department. Violation of this ordinance can be up to a $500 fine. If wishing to obtain a permit please down the form here and present to the police department for approval.
